Personally I think it's because previous ones were subtle; I mean, I just noticed it! HIAM, TTWE and Perfume were really obvious! Not to mention her swollen face during the GMA performances

Britney's face is so unique and pretty, even noticeable small changes can throw everything off, imo. Like in Stronger and Do Somethin' her lips look more plump, but not of a new shape like in Perfume.And the puffy face probably come from medication. But yeah, ever since 2011, her face has been....on a rollercoaster ride it seems. And I may sound mean, but that's just what I see.

Because she didn't have injections, it was all makeup, lip liner, highlighting and shading. She was the master of using makeup to create illusions (and push up bras!!) I don't know what back alley plastic surgeon she went to for her lips, but it ruined her Cupid's bow and ruined the look of her lips.

Makes total sense. Britney could have just left well enough alone with her level of beauty and just stuck to contouring and other tricks. Now her lips are obviously different.
